Improper clutch plate bolt holes.
The clutch plate bolt holes are misthreaded, when attempting to remove the bolts it cross threaded destroying two bolts. If I had a number to contact the manufacturer, I would do so. Make sure you run a tap in the pressure plate mount holes, to remove any burs and test fit your blots prior to install.
